This section displays and allows the user to edit the demographic information for the chart.

This sections displays the facility and audit the chart is assigned to.

Patient Demographics

2. Patient Demographics
This section displays the patient's name, sex, and age. Sex and age are editable.

Visit Dates

3. Visit Dates
This sections displays the start and end dates of the visit for the chart. It automatically calculates the length of stay in days.

Physician Dropdown

4. Physician Dropdown
This control displays the physician for the encounter. All physicians in the facility are available in the dropdown. New physicians can be created by clicking the plus symbol next to the control.

Service Type

5. Service Type
This control allows the auditor to select the service type of the chart. Available choices are "INPATIENT" and "OUTPATIENT".

Financial Class Display

6. Financial Class Display
This control displays the financial class of the chart.

Coder Dropdown

7. Coder Dropdown
This control displays the coder for the chart. All coders in the facility are available in the dropdown. New coders can be created by clicking the plus symbol next to the control.
